Defect identifies the problem that the unit or component is having. It can be left blank if a code for the problem does not exist. You can right-click in the field and select Prompt to select the defect from a list.
Department is the department or shop that will perform the repairs. You can right-click in the field and select Prompt to select the department from a list.
Description is a brief description of the problem; it will be filled in automatically if a defect was selected. If a defect was not selected, you should type in a description here. You can use the Comments option on the Change or Display screen to enter more detailed information, if required.
Employee is the employee who is entering the defect into the system. It defaults to the employee who is signed on. You can click on the Prompt button to select the employee from a list.
Facility is the facility that will repair the unit. You can right-click in the field and select Prompt to select the facility from a list.
Item is the component not currently on an equipment unit that has the defect. Only one of unit or item should be entered. You can click on the Prompt button to select the item from a list.
Odometer Reading is the reading on the unit's odometer when the defect report was made.
Serial # is the unit's or component's serial number. It can be left blank for equipment units, to be filled in automatically by Enrich. You can click on the Prompt button to select the serial number from a list.
Source is the class of person who reported the defect, e.g. driver, mechanic, etc. You can click on the Prompt button to select the source from a list.
Unit is the equipment unit that has the defect. Only one of unit or item should be entered. You can click on the Prompt button to select the unit from a list.
